There is a readme.txt file included in the zip package with the instructions how to install the script.

 For adding the database you have to login to your cpanel, then go to PHPAdmin and upload the file database.txt to the newly created database that it will be filled with the country codes and categories.

Tip:

Before you upload the datanase you also can customize the database.txt file and create your own categories that your new Q & A site will be more niche specific if this is wanted, i.e. you upload the script to a Acne related site and then you rename all categories to something like Acne Scar, Cure Acne etc.
